1	Application to be used	1. In Form 48 as prescribed in C.M.V Rule 2. Application for the grant of authorization should be in Form 46 as prescribed in C.M.V. Rule
2	Records to be filed along with the application	1. Registration Certificate of the vehicle. 2. Fitness Certificate of the vehicle. 3. Insurance certificate of the vehicle. 4. Proof of payment of Tax. 5. Authorization fee of Rs.500/- in the form of Treasury Challan. 6. Demand drafts were drawn in favor of the Authorities prescribed in respect of another state towards payment of composite taxes. 7. Payment of green tax wherever applicable.
